Den svenske zoologen Nils von Hofsten (1881-1967) var en centralgestalt inom svensk eugenik och steriliseringsverksamhet. Han var verksam i närmare femtio år och utövade ett betydande inflytande över steriliseringsverksamheten som lobbyist, vetenskaplig expert och ämbetsman i Medicinalstyrelsen. Denna bok granskar kritiskt hans eugeniska engagemang och dess konsekvenser. von Hofsten tillhörde ett socialt nätverk av rasbiologiskt intresserade akademiker som i gemensamma lobbykampanjer lanserade eugeniken i Sverige. De propagerade för ett rasbiologiskt forskningsinstitut, en steriliseringslag och andra eugeniska reformer. Boken analyserar den syn på mäns och kvinnors olika samhällsroller som framträdde i von Hofstens resonemang och i de eugeniska teorierna och som bidrog till att så många av de steriliserade var kvinnor. Under von Hofstens verksamhetsperiod kom den samhälleliga inställningen till eugenik och steriliseringar att förändras flera gånger. Med tiden riktades allt mer kritik mot steriliseringarna, men von Hofsten stod fast vid att sterilisering av så kallade sinnesslöa var bra för samhället. I boken diskuteras även den "ärftlighetsskräck" som uppstod under 1940- och 1950-talen som en följd av steriliseringsverksamheten. Den yttrade sig i att individer blev skräckslagna för sina egna arvsanlag och inte önskade sätta barn till världen, trots att de ofta var helt friska. von Hofsten fick inom ramen för Medicinalstyrelsen utarbeta strategier för att bemöta och söka förebygga denna starka oro.
